Do you know a manager that has gone above and beyond to ensure the NHS is the best it can be?

Do you know a practice, hospital, NHS Worker or team who has gone out of their way to meet the needs of an excluded or disadvantaged group, or who has worked to tackle discrimination or prejudice in the delivery of healthcare?
